Job summary

Covenant Health is seeking a part-time Nurse Practitioner for Palliative Care to deliver primary care under physician supervision in a homecare/hospice setting. You will work day shifts, serve as a resource for staff, and participate in quality improvement initiatives while following established protocols.

Your role includes patient assessment, history taking, documentation, and coordination with the care team to ensure optimal outcomes for patients at home and in hospice services.

Qualifications

  • Current DEA license.
  • Certification by ANA, AANP or NCC in Family NP/Adult-Gerontology/Psychiatric-MMH as applicable.
  • CPR certification required.

Responsibilities

  • Provides direct patient care under physician supervision and established medical protocols.
  • Maintains patient records accurately and timely.
  • Collaborates with physician preceptor to review medical protocols and adjust care plans.
  • Obtains medical histories, performs exams, orders and analyzes lab work as needed.
  • Promotes health maintenance and patient education; may supervise clinical staff.

Part Time, 48 Hours Per Pay Period, Day Shift

Position Summary:

Provides primary care to patients under the supervision of a physician preceptor, following established medical protocols.

Responsibilities
  • Maintains established departmental policies and procedures, objectives, quality assurance program, and safety standards.
  • Provides direct patient care, evaluates outcomes, consults with other specialists as required, and adjusts processes as indicated to ensure optimal patient care.
  • Collaborates with physician preceptor to formulate and review medical protocols.
  • Obtains complete medical history of the patient.
  • Identifies health hazards in the patient\'s environment as well as patient risk factors. Identifies and considers the cultural patterns, economic status, and patient resources in plan of care.
  • Performs appropriate physical exam as indicated.
  • Obtains appropriate laboratory specimens.
  • Analyzes objective and subjective data for appropriate diagnosis and treatment.
  • Assures treatment and/or plan of care is consistent with established protocols.
  • Monitors and ensures accuracy of recording on patient medical records and maintains all required reports, records, statistics, etc. Documents all pertinent information accurately, legibly, and in a timely manner.
  • Refers patient to appropriate services which exceed scope of practice of the practitioner.
  • Provides health promotion counseling and develops patient teaching information.
  • Serves as a resource person for other staff personnel. May supervise clinical staff.
  • Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.
  • Attends meetings as required and participates on committees as directed.
  • Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.
  • Performs other duties as assigned.
Qualifications

Minimum Education:

None specified; however, must be sufficient to meet the standards for achievement of the below indicated license and/or certification as required by the issuing authority.

None.

Licensure Requirement:

Current DEA license. Must have and maintain certification by the American Nurse Association (ANA), American Academy of Nurse Practitioners (AANP) or National Certification Corporation (NCC) with specialty applicable to the area of responsibility – Family Nurse Practitioner, Adult-Gerontology, Psychiatric-Mental Health. Must meet state licensure requirements. CPR required.
