Job summary

Southcoast Health System, Inc. is seeking an Advanced Practice Practitioner to join the Health at Home - Supportive Care Hospice - Palliative Consulting Team. This full-time, day-shift role offers a structured environment with no weekends, holidays, or on-call.

Responsibilities include collaborative care delivery, patient and family education, and advanced nursing practice in hospice settings. Requirements include an MSN for nursing, NP certification, MA and RI RN licenses, and at least 5 years

Qualifications

MSN required for NP role.
Current NP certification is required.
RN license in Massachusetts and Rhode Island is required.
Minimum 5 years of nursing experience; oncology, hospice, geriatrics or ICU preferred.
Minimum one-year NP experience.
Certification in Hospice and Palliative Care required; CPR certification.

Responsibilities

Collaborate with physician and palliative care team to deliver hospice care and related services.
Perform bedside evaluations, assist with diagnosis, treatment, and management of health conditions, and educate families and caregivers.
