Job summary

Compassus is seeking a Nurse Practitioner to support hospice and palliative care patients across multiple visits and settings. The role emphasizes collaboration with the Medical Director, IDT, and family care teams to provide symptom management, education, and compassionate support.

Applicants should hold an NP license in the state of employment, with hospice/palliative care experience preferred. The position requires flexible scheduling to meet patient needs and after-hours coverage as needed.

Qualifications

• Active CNS or Nurse Practitioner license in the state of employment (required).
• Board Certification in Hospice and Palliative Care is preferred, but not required.
• Experience in hospice or palliative care settings is beneficial.
• Excellent communication and collaboration with IDT and families.

Responsibilities

• Perform routine and emergency assessments for hospice/palliative patients.
• Coordinate care with Medical Director, Attending Physician, DCS and IDT.
• Provide symptom management, education, and support to patients and families.
• Maintain accurate patient records and ensure documentation complies with CMS guidelines.
