Job summary

UnitedHealth Group, via Casa de la Luz Palliative Care in Mesa, AZ, invites you to join as a Palliative Care Nurse delivering direct patient and family care through palliative consultations for patients with serious illness. You will perform comprehensive assessments, apply evidence‑based practices, reconcile medications, and coordinate a care plan with the team.

Required: RN/APRN licensure, prescriptive authority, BCLS, CPR, and 5 years of clinical nursing experience; ACHPN is preferred.

Qualifications

• RN/APRN licensure in state of practice.
• Nurse Practitioner/CNS with prescriptive authority.
• BCLS licensure.
• Minimum 5 years of clinical nursing experience.
• Current CPR certification.

Responsibilities

• Demonstrates ethical use of palliative medicine practices in patient care.
• Incorporates palliative care knowledge to formulate an effective plan of care.
• Gathers pertinent information from medical records, family history, and caregivers and conducts comprehensive assessments.
• Completes a comprehensive assessment utilizing palliative care domains and initiates a plan of care with therapeutic interventions.
• ACHPN certification encouraged after 2 years of employment or when experience requirements are met.
