Job summary

HopeHealth is seeking an experienced Palliative Nurse Practitioner for its affiliate HopeHealth Hospice & Palliative Care to treat patients primarily at Sturdy Memorial Hospital in Attleboro, MA. This full-time role supports 8:00am–4:30pm hours and is eligible for benefits.

The NP collaborates with referring providers, assesses and relieves symptoms, and helps patients and families set compassionate goals for care. MA/RI licenses are required.

Qualifications

• Master’s degree in Nursing or Doctorate in Nursing required.
• Certified as a Family/Geriatric or Adult NP and RN Practitioner in RI/MA.
• 2+ years in geriatrics, hospice, or oncology preferred.
• Experience with pain and symptom management is highly preferred.

Responsibilities

• Direct patient care for seriously ill adults in inpatient and outpatient settings.
• Complete pain assessments and pain management recommendations.
• Manage non-pain symptoms (nausea, fatigue, dyspnea, anxiety).
• Assess disease trajectory with prognosis and treatment recommendations.
• Collaborate with referring providers to improve patient outcomes.
• Counsel patients and families on goals of care and end-of-life options.
• Coordinate care with patients, families, and facilities; support education.
