Job summary

Reserve Care is seeking a Nurse Practitioner to lead palliative and hospice care across settings, focusing on relief of suffering, comfort, and quality of life for patients and families. The role emphasizes education, collaboration, and evidence-based practice in a multidisciplinary environment.

The NP will deliver advanced nursing care, participate in program development, and engage with hospitals, home care, and long-term care settings to advance palliative care standards in Ohio.

Qualifications

• Maintains current knowledge of palliative care practices and guidelines.
• Educates patients and families on palliative care options and goals of care.
• Provides education to HWR staff on palliative care principles.
• Maintains and cultivates professional relationships to support the hospice mission.
• Active involvement in professional organizations, presentations and publications.

Responsibilities

• Develops and implements new programs affecting clinical practice.
• Performs holistic patient assessments and prescribes symptom management as appropriate.
• Reviews medical history and labs to form a care plan with physicians.
• Coordinates care with the interdisciplinary team to support patients and families.
• Provides independent advanced practice nursing care within Ohio scope of practice.
• Contributes to quality improvement activities and committee work.
