Job summary

Ohio Living in Cleveland seeks a Nurse Practitioner to provide preventive medical, palliative, and educational services to patients across home health and hospice programs. You will comply with state and local standards using our comprehensive clinical-financial computer system and EMR software to deliver high‑quality care with integrity and compassion.

You will assess health histories, manage illnesses, prescribe appropriate therapies, coordinate referrals, and contribute to quality improvement

Qualifications

Current unencumbered Ohio CNP license.
Current prescriptive authority certificate or externship certificate to prescribe.
Minimum two years of experience in home health and/or hospice.
Proficiency with Windows, Microsoft Office, and the internet.
Must read/write/speak English proficiently.

Responsibilities

Conducts comprehensive medical and social histories and performs physical examinations.
Treats common acute illnesses, chronic and stable health conditions.
Orders and interprets lab tests and x-rays.
Provides information, instruction, and counseling on health maintenance and disease management.
Refer patients to other health professionals as needed.
Communicates with staff, patients and families to promote optimal outcomes.
Completes documentation accurately and timely per regulations.
Participates in quality improvement projects.
Commitment to excellent customer service for internal and external customers.
