Job summary

Reserve Care is seeking an experienced Registered Nurse to provide compassionate hospice and palliative care in Ohio. You will assess patient and family needs, coordinate care plans, and support a multidisciplinary team to ensure comfort and quality of life.

Qualifications

• Must be a licensed RN in the state of Ohio.
• Current CPR certification required.
• Must have a valid driver’s license and good driving record.

Responsibilities

• Provide intermittent skilled nursing services under physician direction.
• Assess physical, psychosocial, and spiritual needs of patients and families.
• Monitor and manage pain and symptoms; educate patient and family.
• Coordinate Plan of Care with transdisciplinary team.
• Serve as liaison to physicians with patient information as needed.
• Provide emotional support to patient and family.
• Refer patients/family to other services and follow up as necessary.
• Provide health teaching to patient and family.
• Maintain and complete hospice-related plans and reports.
• Participate in care conferences and bereavement follow-up.
• Participate in case reviews and quality improvement activities.
• Supervise activities of Nursing Assistants related to patient care.
