Job summary

AlignMed Partners is seeking a full-time Palliative Care Nurse Practitioner to float between our Pittsburgh centers (Monroeville Post Acute, Bridgeville Rehabilitation & Care, Ivy Park Post Acute). The base salary is $114,000–$120,000 per year, with a no-cap incentive compensation program and no weekend on-call requirements.

Qualifications

• Graduate from an accredited School of Nursing, RN licensed, and NP certification with MSN and geriatric/older adult focus.
• Two years of adult/geriatric population experience preferred.
• APRN with unrestricted license and DEA in practicing states.

Responsibilities

• Provide primary and urgent care to nursing center residents in collaboration with the medical team.
• Deliver palliative care with emphasis on pain management, symptoms, psychosocial support, and advance care planning.
• Support ongoing education for staff and participate in quality initiatives and compliance programs.
• Adhere to coding, billing, and AlignMed policies; collaborate with leadership and care teams.
