This role provides primary and urgent care to nursing center residents in collaboration with the leadership team and Medical Director. The Advanced Practice Provider reports to the Director of Clinical Practice and collaborates on-site with center leadership to achieve center goals and priorities.
Qualifications
- Requires NP with MSN and state licensure; NCCPA certification preferred.
- Experience with adult/geriatric populations is valued.
- APRNs must have DEA, if applicable for practice states.
Responsibilities
- Provide primary and urgent care to nursing center residents in collaboration with other providers.
- Collaborate on-site with Center Leadership and Medical Director to meet center goals.
- Support mission to improve patient outcomes with compassionate care.
