Job summary

Community Hospice is seeking an Advanced Practice Registered Nurse to collaborate with physicians and the interdisciplinary team in delivering comprehensive palliative and hospice care. The APRN will assess, treat, and manage patients with serious or terminal illnesses, lead goals-of-care discussions, prescribe medications, and support transitions of care across settings.

PRN as needed for Day Shift, with responsibilities including care planning, documentation, staff education, and coordination

Qualifications

• APRN with strong pain and symptom management knowledge.
• Experience in hospice or palliative care preferred.
• Excellent communication with patients, families and staff.

Responsibilities

• Provide medical expertise in pain and symptom management and prognosis assessment.
• Act as attending provider in hospice contracts with the hospice physician.
• Coordinate admissions/discharges for hospice beds and review diagnoses.
• Develop care plans, implement interventions and evaluate outcomes.
• Educate staff and families about palliative and hospice care.
