Job summary

Community Hospice and Palliative Care in Gainesville, FL seeks an Advanced Practice Registered Nurse to provide comprehensive palliative and hospice care with physicians and interdisciplinary teams. The APRN conducts clinical evaluation, treatment planning, goal-of-care discussions, and care transitions for patients with serious or terminal illnesses.

Qualifications

• Current Florida APRN license or eligibility.
• Experience in hospice or palliative care preferred.
• Ability to conduct goals-of-care discussions and care planning.

Responsibilities

• Provide pain and symptom management and terminal prognosis evaluation.
• Act as attending provider for hospice contract beds with hospice physician.
• Coordinate admissions and discharges for hospice beds.
• Develop care plans, implement interventions, and evaluate outcomes.
• Educate staff, patients, and families about palliative care.
• Maintain accurate medical records and informed consent discussions.
• Participate in interdisciplinary team meetings and on-call rotations.
• Coordinate inpatient APRN assignments and department functions.
