Job summary

Community Hospice & Palliative Care in Florida seeks an Advanced Practice Registered Nurse to join our clinical team. The APRN collaborates with physicians and interdisciplinary colleagues to deliver comprehensive palliative and hospice care, including evaluation, treatment and care management for patients with serious or terminal illnesses, and to lead goals‑of‑care discussions.

PRN hours M-F with weekend availability as needed; role includes education of staff and families, on‑call rotations,

Qualifications

• Master’s degree from an accredited Advanced Practice Nursing program.
• Current Florida Advanced Practice Nurse license.
• Two years nursing experience; hospice or palliative care preferred.

Responsibilities

• Provide clinical evaluation and management of palliative patients.
• Prescribe treatments and manage care plans.
• Educate staff and families about palliative care.
• Collaborate with hospice physicians and interdisciplinary teams.
• Coordinate admissions and care transitions.
• Maintain complete medical records and ensure compliance.
