Job summary

AdventHealth is seeking a licensed Advanced Practice Nurse for Palliative Care at Porter in Denver, CO. The role provides independent NP/PA care, conducts comprehensive assessments, develops treatment plans, educates patients and families, and collaborates with an interdisciplinary team.

Requires a Bachelor's degree in Nursing, a Master's degree in Nursing, BLS, and current Colorado Advanced Practice Registry.

Qualifications

• Palliative care experience desired - inpatient or outpatient.
• Bachelor's degree, Nursing - Required.
• Master's degree, Nursing - Required.
• Basic Life Support - American Heart Association - Required

Responsibilities

• Provides independent advanced practice nursing care including assessment, diagnosis, development of treatment plan including outcome identification, patient/family education, and implementation and evaluation of the treatment plan.
• Performs patient rounding with an interdisciplinary team and participates in case conferences.
• Evaluates patient's response and intervenes to ensure optimal patient outcomes
