AdventHealth is seeking a licensed Advanced Practice Nurse for Palliative Care at Porter in Denver, CO. The role provides independent NP/PA care, conducts comprehensive assessments, develops treatment plans, educates patients and families, and collaborates with an interdisciplinary team.

Requires a Bachelor's degree in Nursing, a Master's degree in Nursing, BLS, and current Colorado Advanced Practice Registry.

Qualifications

• Palliative care experience desired - inpatient or outpatient.
• Bachelor's degree, Nursing - Required.
• Master's degree, Nursing - Required.
• Basic Life Support - American Heart Association - Required

Responsabilités

• Provides independent advanced practice nursing care including assessment, diagnosis, development of treatment plan including outcome identification, patient/family education, and implementation and evaluation of the treatment plan.
• Performs patient rounding with an interdisciplinary team and participates in case conferences.
• Evaluates patient's response and intervenes to ensure optimal patient outcomes

AdventHealth Medical Group (AHMG} is a sister organization of AdventHealth which employs 400+ physicians in 25+ individual service lines in the Rocky Mountain Region. This aligned model and practice management experience provides the opportunity for physicians to focus on the clinical aspects of medicine. Responsibilities

Provides independent advanced practice nursing care including but not limited to completions of assessment, diagnosis, development of treatment plan including outcome identification, patient/family education, and implementation and evaluation of the treatment plan. Performs patient rounding with an interdisciplinary team and participates in case conferences. Performs comprehensive physical, psychological, social, spiritual, and cultural assessments through observation, interview, review of medical records and clinical data inclusive of imaging and lab results. Evaluates patient's response and intervenes to ensure optimal patient outcomes

Qualifications

• Palliative care experience desired - inpatient or outpatient
• Bachelor's degree, Nursing- Required
• Master's degree, Nursing- Required
• Basic Life Support-American Heart Association - Required
• Registered as Advanced Practice Nurse - Required
• Registered on the State of Colorado Advanced Practice Registry - Required
