Aultman Health System in Canton, OH seeks an Advanced Practice Provider (CNP/APN) for Home Palliative Care. The role focuses on comprehensive assessments and pain/disease management in home and nursing facility visits.

You will collaborate with Hospice and Palliative Care physicians and support a 60-80 patient panel. Requirements include APRN or PA certification, current licensure, DEA, and 3 years in nursing with hospice experience preferred, plus BLS.

Qualifications

• Successful completion of advanced practitioner training and certification from an approved nurse practitioner or physician assistant program.
• Current state license as an APRN with prescriptive authority or PA license with DEA certification.
• Three years of full-time nursing experience; hospice nursing preferred.
• Excellent interpersonal and written/oral communication; strong organizational and problem-solving skills.
• Current BLS certification.

Responsabilités

• Provide advanced comprehensive assessments and develop treatment plans for hospice and palliative care patients.
• Document assessments, diagnoses, and management of care.
• Prescribe medications per state regulations and Aultman policies.
• Collaborate with hospice physicians for consultations and patient rounds.
• Maintain continuity of care with the interdisciplinary team and community partners.
• Participate in QAPI and quality improvement activities.
