AdventHealth in Denver, CO is seeking a Palliative Care Advanced Practice Provider (APP) to join AdventHealth Medical Group at Porter. You will model clinical excellence, partner with an interdisciplinary team, and contribute to compassionate, whole-person care.

This full-time Day shift role offers an annual compensation range of $133,435 to $142,821, with comprehensive benefits and relocation assistance. Join a mission-driven system focused on Extending the Healing Ministry of Christ.

Formación

• Palliative care experience desired - inpatient or outpatient
• Bachelor's degree, Nursing- Required
• Master's degree, Nursing- Required
• Basic Life Support-American Heart Association - Required
• Registered as Advanced Practice Nurse - Required
• Registered on the State of Colorado Advanced Practice Registry - Required

Responsabilidades

• Provides independent advanced practice nursing care including assessments, diagnosis, and treatment planning.
• Performs patient rounding with an inter-disciplinary team and participates in case conferences.
• Performs comprehensive physical, psychological, social, spiritual, and cultural assessments.
• Evaluates patient responses and intervenes to ensure optimal outcomes.

AdventHealth Medical Group (AHMG) is a sister organization of AdventHealth which employs 400+ physicians in 25+ individual service lines in the Rocky Mountain Region. This aligned model and practice management experience provides the opportunity for physicians to focus on the clinical aspects of medicine. Responsibilities

Job Description:

• Provides independent advanced practice nursing care including but not limited to completions of assessment, diagnosis, development of treatment plan including outcome identification, patient/family education, and implementation and evaluation of the treatment plan.
• Performs patient rounding with an interdisciplinary team and participates in case conferences.
• Performs comprehensive physical, psychological, social, spiritual, and cultural assessments through observation, interview, review of medical records and clinical data inclusive of imaging and lab results.
• Evaluates patient's response and intervenes to ensure optimal patient outcomes.

Qualifications

• Palliative care experience desired - inpatient or outpatient
• Bachelor's degree, Nursing- Required
• Master's degree, Nursing- Required
• Basic Life Support-American Heart Association - Required
• Registered as Advanced Practice Nurse - Required
• Registered on the State of Colorado Advanced Practice Registry - Required
