Job summary

Community Counseling of Bristol County in Brockton, MA is seeking an entry-level Case Manager for the PACT-Y program. You will work with a multi-disciplinary team to support youth transitioning to adulthood, helping with community resources, jobs, after-school activities and treatment goals.

The role requires a Bachelor’s degree, valid driver’s license, and willingness to learn. It offers supervision, growth opportunities and a commitment to serving families in southeastern Massachusetts.

Qualifications

• Bachelor’s degree in psychology, social work or a related field.
• Some prior experience working with children, youth and families with mental health challenges.
• A valid driver’s license and reliable transportation.
• Willingness to learn and seek guidance as needed.
• Good organizational skills, in addition to effective verbal, written and computer skills.

Responsibilities

• Provide case management services to increase skills related to independent living, community integration, employment, recreation and housing.
• Assist families in obtaining resources to improve family functioning within their community.
• Support and teach skills related to the treatment plan.
• Model skills and techniques in the community to increase youth’s feelings of competence and self-esteem.
• Ensure the youth has a voice in treatment planning and identification of goals.
• Complete required documentation per program guidelines.
• Coordinate care with the family, team, providers and DMH.
• Provide on-call and weekend support on a rotating basis.
• Facilitate change based on youth and family needs, strengths and preferences.
