Job summary

Clinical & Support Options, Inc. is seeking a Case Manager to join the PACT team in Greenfield, MA. The role provides intensive case management to adults 18+ to help maintain independence in the community and engage those difficult to reach in behavioral health services.

You will offer motivational counseling, crisis and transition support, and assist clients with daily living skills, housing, and access to community resources, while collaborating with a mini-treatment team.

Qualifications

• Bachelor's degree in a behavioral health discipline or related field.
• Must be able to work flexible schedule which may include evening and weekend work, as required by client needs.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide intensive case management to adults 18 years and older to support independence in the community.
• Provide motivational counseling, crisis intervention, and transition support during transitions.
• Identify and connect clients with mental health, addiction, and medical services.
• Develop and support daily living skills, communication, conflict resolution, and community integration.
• Assist in locating and maintaining housing and coordinating with landlords and housing authorities.
