Job summary
Elwyn is seeking a Team Leader to join the Program for Assertive Community Treatment (PACT) in Raleigh, NC. The role combines clinical practice with supervision, guiding a team to monitor client status and deliver evidence-based services.
The successful candidate will be licensed in NC (LCSW, LICSW, or LMHC) with 5+ years in leadership and a passion for serving adults with mental health challenges. Competitive pay and benefits are provided.
Qualifications
- Licensed in the State of North Carolina as LCSW, LICSW, or LMHC.
- 5+ years' leadership experience, preferably with adults with mental health issues.
- Experience delivering or supervising Assertive Community Treatment (PACT) services.
- Must possess a valid driver’s license and a clean driving record.
Responsibilities
- Direct day-to-day clinical operations of the PACT team, including scheduling staff and leading meetings.
- Coordinate admissions and develop Comprehensive Assessments and Community Service Plans.
- Provide clinical supervision and crisis intervention as needed.
- Monitor client progress and adjust Community Service Plans accordingly.
- Ensure adherence to program policies, documentation, and EHR requirements.
- Support staff recruitment, onboarding, and performance supervision.
