Qualificações

• Must be a graduate of an approved nursing program and licensed to practice in the United States.
• English language proficiency required for direct patient care per VA standards.
• Preparation for state licensure and ability to obtain permanent appointment after bridge program completion.

Responsabilidades

• Assess, plan, implement, and evaluate care for Veterans in a Primary Care outpatient clinic.
• Coordinate care with the Patient Aligned Care Team (PACT) and interdisciplinary staff.
• Administer medications and procedures per established policies.
• Provide health promotion, education, and coaching on wellness and chronic disease management.
• Support safe transitions of care and patient self-management across the continuum.
