Job summary

Qualifications

• English language proficiency per 38 U.S.C. 7403(f).
• Graduate of a school of professional nursing approved by accrediting bodies (ACEN/CCNE).
• Current, full, active, unrestricted RN license in the United States.

Responsibilities

• Provide comprehensive, patient-centered nursing care in a primary care setting.
• Assess, plan, implement and evaluate care with a focus on education and chronic disease management.
• Coordinate care with interdisciplinary team to improve patient outcomes and continuity of care.
