• Foreign educated nurses require degree equivalency validation.

Responsibilities

• Provide fundamental primary care coordination for the PACT and transition of care.
• Collaborate with interdisciplinary team to deliver Veteran-centered nursing care.
• Apply evidence-based guidelines and population health tools for high-risk patients.
• Coordinate care across acute and ambulatory settings and post-discharge transitions.
• Engage in health coaching and chronic disease management.
