Job summary

The U.S. Department of Veterans Affairs is seeking a Patient Aligned Care Team (PACT) Registered Nurse Care Manager for the Montevideo Community Based Outreach Clinic to provide coordinated, nurse-led care in outpatient primary care.

The RNCM will work with the PACT team to manage panels of Veterans, emphasizing preventive care and chronic disease management. Ideal candidates will have an RN license, nursing degree from an accredited program, and demonstrated ability to collaborate within a

Qualifications

English language proficiency required for direct patient care.
Graduate of a nursing program with approved accreditation.
Current, full, active nursing license in one US state or territory.
Eligibility for licensure transfer or bridge programs may apply.
Basic knowledge of care coordination and patient education.

Responsibilities

Manages care coordination for a designated panel of patients focusing on health promotion and chronic care.
Collaborates with PACT team and external services to ensure holistic, patient-driven care.
Educates patients and families; advocates for patient needs and outcomes.
Leads transitions of care, coordinating tests, referrals, and post-discharge follow-up.
Uses data to identify high-risk patients and coordinate appropriate referrals.
