Qualifications

• Graduate of a school of professional nursing approved by accrediting agency and accredited by ACEN or CCNE.
• Current, full, active nursing license in the United States or DC.
• Evidence of nursing education meeting VA requirements for appointment as RN.
• Foreign-educated nurses require degree equivalency validation.

Responsibilities

• Coordinate and manage care for an assigned patient panel.
• Lead chronic disease management, preventive care, and health promotion efforts.
• Identify high-risk patients using population health tools and coordinate transitions of care.
• Provide patient and family education, health coaching, and self-management support.
• Collaborate with interdisciplinary teams to improve access, continuity, quality and patient experience.
