Job summary

University of Minnesota School of Nursing seeks a Patient Aligned Care Team (PACT) Registered Nurse Care Manager for the Montevideo Community Based Outreach Clinic. The RNCM delivers comprehensive, coordinated nursing care within the primary care PACT framework and engages in care planning and self-directed teamwork.

The role emphasizes care coordination, patient education, and collaboration with the extended care team to support Veterans’ wellness and transitions of care.

Qualifications

• Graduate of a professional nursing program approved by a recognized accrediting agency.
• Current, full, active RN license in the United States.
• English language proficiency required for direct patient care.
• Experience in care coordination and chronic disease management is preferred.

Responsibilities

• Manages care coordination for a designated patient panel emphasizing health promotion and chronic care management.
• Collaborates with PACT and external services to ensure holistic, patient-driven care and timely access to resources.
• Educates patients and families; advocates for patient needs to improve outcomes and wellness.
• Leads a collaborative team approach to ensure continuity and quality of care tailored to patient goals.
• Oversees transitions of care, proactive pre-visit prep, labs, and referrals to support transitions and follow-up.
