U.S. Department of Veterans Affairs is seeking a Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) at the Fort Smith VA Clinic. The role includes direct Veteran care and care coordination within Primary Care, with occasional travel to the Ozark VA Clinic depending on team assignment.

The RN must meet basic nursing education requirements, obtain and maintain licensure, and demonstrate leadership and evidence-based practice in a primary care setting.

Qualificações

• English language proficiency required.
• Graduate of an approved nursing program (ACEN/CCNE).
• State license and eligibility for temporary/permanent appointment following licensure.
• Foreign nursing education requires equivalency validation.

Responsabilidades

• Provides direct Veteran care and care coordination within Primary Care.
• Leads and coordinates care for assigned patient panels using registries and evaluation processes.
