Job summary

The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse at the Dayton VAMC provides direct Veteran care and care coordination within the Primary Care Department. The RN manages assigned panels, uses registries, and delivers health promotion, education, coaching on wellness and chronic care management to Veterans.

The role requires a nursing degree from an accredited program, an active RN license in the United States, and the ability to lead care teams.

Qualifications

• Graduate of a nursing program approved by an accrediting body (ACEN or CCNE) at time of completion.
• Current, full, active, unrestricted RN license in a U.S. state/territory (including DC).
• Bridge-program pathway may allow licensure prior to completion in select cases.
• Foreign-educated nurses require degree equivalency validation per VA policy.

Responsibilities

• Provide competent, evidence-based nursing care to primary care patients.
• Oversee licensed vocational nurses and nursing assistants as appropriate.
• Lead initiatives to improve Veteran/patient experience, satisfaction and safety beyond the immediate setting.
• Communicate and collaborate effectively as a leader of the interprofessional team.
• Support care coordination, registries, and chronic care management within PACT.
