Job summary

The U.S. Department of Veterans Affairs seeks a Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse at the Dayton VAMC. This role provides direct Veteran care, care coordination, and oversight of licensed staff within the Primary Care Department.

Responsibilities include applying nursing process, health promotion, education, and chronic care management to Veterans across assigned panels, with potential involvement in quality improvement beyond the immediate practice setting.

Qualifications

• English language proficiency required for direct patient care.
• Graduate of a nursing program approved by a recognized accrediting body (ACEN/CCNE).
• State nursing license and ability to obtain licensure in US territories as applicable.

Responsibilities

• Provide competent, evidence-based nursing care to Veterans.
• Coordinate care and oversight of licensed nursing staff.
• Promote health, wellness education and chronic care management in primary care.
