Job summary

Veterans Health Administration, Richard L. Roudebush VAMC Indianapolis, Indiana, seeks a Staff Registered Nurse for the Primary Care Service to join the Patient Aligned Care Team (PACT).

The RN will provide gender-specific veteran care, care coordination, and leadership within a complex primary care environment. The role emphasizes delivering patient-centered care, coordinating with interprofessional teams, and contributing to performance improvement in outpatient settings.

Qualifications

• English language proficiency required for direct patient care.
• Graduate of a nursing program accredited by ACEN or CCNE.
• State licensure as a nurse is required; foreign graduates need equivalency validation.

Responsibilities

• Manages assigned panel(s) and ensures access for patients in Primary Care/PACT.
• Provides health promotion, wellness coaching, chronic care management.
• Collaborates in development and evaluation of policies and guidelines.
• Demonstrates leadership and supports peers in professional development.
• Ensures documentation of PACT program outcomes at service level.
• Works to maintain safe, high-quality nursing care in a busy outpatient setting.
