Job summary

Qualifications

• English language proficiency required for direct patient care.
• ACEN- or CCNE-accredited nursing program graduate.
• State licensure upon graduation and eligibility.
• Foreign-educated nurses require degree equivalency validation.
• Physical requirements and ability to perform nursing duties.

Responsibilities

• Lead proactive, patient-centered care using established nursing protocols.
• Coordinate care for designated patient panel with emphasis on education and self-management.
• Collaborate with PACT team to ensure timely, coordinated access to care resources.
• Educate patients and families to support wellness and self-management goals.
• Promote continuity and quality of care within the practice setting.
