Job summary

U.S. Department of Veterans Affairs is seeking a self-directed Registered Nurse (RN) to serve as a Patient Aligned Care Team (PACT) Care Manager. You will provide comprehensive, coordinated care for primary care patients in an interdisciplinary, team-based setting.

The role emphasizes care coordination, patient education, and transitions of care, with shifts scheduled Monday-Friday, 7:30 am–5:00 pm. Intermittent employment and non-telework are specified.

Qualifications

• Must be proficient in English (written and spoken).
• Graduate of a nursing program approved by an accrediting body (ACEN/CCNE).
• Licensed as a Registered Nurse in the United States and eligible for state licensure.
• May include bridge program options and temporary appointment provisions as described.

Responsibilities

• Functions as a team member providing comprehensive, coordinated nursing care to panels of patients with occasional supervision.
• Manages care coordination for a designated panel of patients, emphasizing health promotion and chronic care management.
• Collaborates with PACT members and external services to develop holistic, patient-driven care and ensure timely access to resources.
• Engages in patient and family education and advocacy to improve outcomes.
• Oversees transitions of care and supports Veteran-centered nursing across multiple modalities.

Education

Associate Degree in Nursing or Diploma in Nursing

Bachelor of Science in Nursing (BSN)

Master's degree in nursing (MSN) or related field with BSN

State RN license (active, unrestricted)
