Job summary

Qualifications

• English language proficiency for direct patient care (written and spoken).
• Graduate of a school of professional nursing approved by an accrediting agency (ACEN/CCNE).
• Current, full, active, and unrestricted nursing license in the United States or equivalent.
• GNT exception allowed: temporary appointment while pursuing licensure.
• Graduate education in nursing or related pathways may meet requirements if licensure is obtained.

Responsibilities

• Provide nursing care and care coordination within the Primary Care PACT.
• Coordinate care focusing on patient education, self-management and patient satisfaction.
• Act as a key team member delivering comprehensive, coordinated nursing care.
