Job summary

The U.S. Department of Veterans Affairs is seeking a self-directed Registered Nurse (RN) to join the Patient Aligned Care Team (PACT) as a Care Manager.

You will provide comprehensive and coordinated care for primary care patients within an interdisciplinary, team-based environment. Basic requirements include English proficiency, graduation from an approved nursing program, and current, full, active RN licensure in the United States or equivalent; a temporary GNT option exists for licensure

Qualifications

• English language proficiency required for direct patient care.
• Education must be from an approved nursing program (ACEN/CCNE) or equivalent bridge pathway.
• Current, full, active, unrestricted RN licensure in US territory or DC; GNT exception may apply.

Responsibilities

• Provide comprehensive, coordinated care for primary care patients in an interdisciplinary team-based setting.
• Contribute to patient care planning and continuity within the Patient Aligned Care Team (PACT).
• Comply with VA qualification standards and pre-employment requirements including licensure and English proficiency.
