Diversity Group is seeking an experienced RN Care Manager for the PACT 1 team at the Camdenton CBOC, to deliver patient-centered nursing care in an outpatient setting.

The role requires current RN licensure, English proficiency, and completion of an ACEN/CCNE-accredited nursing program or equivalent bridge pathway. Compliance with VA nursing guidelines is essential for appointment and progression.

Qualifications

  • RN must be licensed and eligible for VA nursing standards.
  • Nursing education from ACEN or CCNE accredited program.

Responsibilities

  • Provide direct patient care as part of the PACT team.
