Job summary

Veterans Health Administration is seeking a Registered Nurse - Patient Aligned Care Team (PACT) Care Manager to deliver and improve holistic care through collaboration with team members. You will develop patient-driven care plans, monitor outcomes, and educate patients and families on self-management and wellness.

The role emphasizes leadership in chronic care management, coordination of services, and adherence to VA standards. A BSN is preferred with a strong focus on comprehensive nursing care.

Qualifications

• English language proficiency required for direct patient care.
• Current full, active, unrestricted nursing license in US/DC required.
• Graduate of a nursing program approved by accrediting agencies (ACEN/CCNE) or equivalent.
• Foreign nursing education requires degree equivalency validation.
• Bridge program pathways may apply for RN licensure before completion.

Responsibilities

• Collaborate with the care team to implement patient-driven care plans.
• Deliver holistic nursing care and disease management per care standards.
• Collect data and apply evidence-based changes to practice based on results.
• Facilitate care transitions with internal and external services.
• Provide patient and family health education focusing on self-management, prevention and wellness.
