Qualifications

• Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by ACEN or CCNE
• Current, full, active, and unrestricted registered nurse license in a U.S. state or territory
• English language proficiency as required by 38 U.S.C. 7403(f)

Responsibilities

• The RN Care Manager is a member of PACT and will function as an integral member of the interdisciplinary team in managing chronic diseases
• The RN must demonstrate outstanding communication skills with the ability to triage patient concerns and complaints in person or via telephone
• The RN applies the principles of advanced clinic access and PACT principles within daily work
• The RN demonstrates advanced clinical expertise in the nursing process and applies all aspects of the nursing process within a collaborative practice setting
• The RN promotes health, disease prevention, and management of acute and chronic illness
