Responsibilities

• The Registered Nurse (RN) - Patient Aligned Care Team (PACT) Care Manager works in partnership with an assigned panel of patients and team members for effective disease and chronic care management.
• They use the nursing process and evidence-based practice to collaborate with the expanded team, internal and community-based services, in developing the patient driven care plan for prevention and management of diseases.
• Major duties include data collection and implementing evidence-based changes to practice based on data results.
• Collaborates with VA internal and external services to facilitate care transitions to meet patient needs.
• Provides evidence-based patient and family health education with a focus on self-management, prevention, and wellness.
• Initiates follow-ups as recommended while administering prescribed therapies, and performs other duties as assigned to support the agency.
