The Licensed Practical Nurse (LPN) position is in Primary Care at the Grand Forks VA, part of a VHA Patient Aligned Care Team (PACT). The LPN performs specialized nursing activities under the direction of a Registered Nurse, using clinical judgment to make nursing care decisions with minimal supervision.

Responsibilities include medication administration, patient and family education, and collaboration with RNs, MDs, and health technicians to provide comprehensive, accessible and compassionate

Formación

• Graduate of a school of practical or vocational nursing approved by the State or NLNAC-accredited at program completion.
• Active, full, current and unrestricted LPN licensure in US territory or DC.

Responsabilidades

• Join a team providing care to adult and geriatric patients under RN/MD supervision.
• Administer prescribed medications and document actions per policy.
• Provide patient and family education on disease processes, medications, and treatment plans.
• Collaborate with RNs, providers, Nursing Assistants and Health Technicians to deliver comprehensive care.
• Assist in emergency situations and reporting patient condition changes to RN/MD.
