Job summary

Clinical & Support Options, Inc. (CSO) is seeking a PACT Case Manager in Northampton for a full-time, Monday–Friday role (8:30am–4:30pm) focused on intensive case management to support adults with behavioral health needs and reduce hospitalizations.

The position involves facilitating housing, employment, SNAP/SSI/SSDI applications, coordinating resources, daily living skills training, and transportation within a multidisciplinary team.

Qualifications

• Bachelor's degree in a behavioral health discipline or related field.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide motivational counseling, prompting, and intensive support to consumers during psychiatric crises or periods of transition; monitor and assist clients with grocery shopping, meals, and household activities.
• Identify and connect clients with community resources including mental health, addiction and medical services.
• Provide skills training in daily living activities and communication, conflict resolution, and goal development with individualized placements.
