Job summary

St. Paul’s PACE Corp in San Diego, CA is seeking an IHS Staffing Coordinator to manage daily caregiver schedules and ensure services align with each participant's plan of care.

Under the supervision of the Clinical Services Supervisor, you will attend IDT meetings, coordinate IHS services, organize efficient driving routes, and maintain clear communication with participants, families, and staff. Weekend and on-call rotations are involved.

Qualifications

• One year of scheduling or administrative experience preferred.
• Experience with electronic medical records (EMR) and scheduling software.
• Excellent communication and organizational skills.

Responsibilities

• Attend Interdisciplinary Care Team (IDT) and plan of care (POC) daily meeting.
• Schedule IHS services for all PACE participants and respond to IDT needs.
• Coordinate IHS services for PACE participants for timeliness and efficiency.
• Prepare caregiver schedules and modify, cancel or reschedule as needed.
• Review and audit daily visits and prepare reports.
• Maintain positive relationships with participants, families, caregivers, staff and organization.
• Maintain records and ensure policies are followed.
• Report issues promptly with possible solutions to Management.
• Perform other duties as assigned.
