Legacy Healthcare is seeking a compassionate Social Worker to join our PACE program, helping older adults live independently and with dignity in the community they love. You will conduct psychosocial assessments, develop care plans with an interdisciplinary team, support families through transitions and grief, connect participants with community resources, and advocate for holistic, person-centered care.

This role requires licensure (LMSW/LCSW) and a passion for serving vulnerable populations,

• Excellent communication and problem-solving skills
