Job summary

Legacy Healthcare in Louisville is seeking a Social Worker for our PACE program to help older adults live independently with dignity. You will assess needs, develop care plans, and connect participants with supportive services.

As part of an interdisciplinary team, you’ll support families through transitions, grief, and social challenges, advocate for resources, and promote holistic care. A passion for serving vulnerable older adults is essential.

Qualifications

• LMSW or LCSW license (required)
• Experience in geriatric, community health, or long-term care settings preferred
• Excellent communication and problem-solving skills
• A passion for serving older adults and vulnerable populations

Responsibilities

• Provide psychosocial assessments and ongoing support to PACE participants and families
• Collaborate with an interdisciplinary care team to create and implement individualized care plans
