Job summary

Element Care in Massachusetts seeks a Social Worker to plan, implement and evaluate care plans under the PACE model, helping older adults live safely at home. The role is full-time, Monday to Friday, 8am to 4pm, with frequent local travel.

The successful candidate will participate in care planning, provide counseling and coordinate hospital or nursing facility admissions as part of a collaborative team.

Qualifications

• Current Social Work licensure in Massachusetts at the Masters level (LICSW or LCSW) required.
• Minimum 1 year experience in Social Work providing traditional Clinical or Case Management services with a geriatric population.
• CPR Certification or ability to become certified.
• COVID vaccinated preferred.

Responsibilities

• Participates in IDT initial assessments, care planning and ongoing reassessments; attends meetings and helps develop plan of care.
• Completes assessments, provides counseling, coordinates hospital admissions/discharges, and ensures PASRR documentation.
