Job summary

Chapters Health System in Florida is seeking an experienced RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team, and deliver clinical, palliative, and supportive care in the patient’s home.

You will conduct home visits, document in the EMR, communicate with physicians and IDT, and ensure a smooth care continuum while upholding safety and compassionate care.

Qualifications

• Current RN license in Florida.
• Minimum 1 year nursing experience in hospice or hospital settings.
• Experience with EMR/EHR systems.
• Valid driver's license and auto insurance.
• Ability to travel to patient homes and perform home safety assessments.
• BLS certification required.
• Strong English communication skills.

Responsibilities

• Provides and manages direct care to patients and families as part of IDT, with psychosocial, spiritual, cultural and physical components.
• Coordinates the Plan of Care to reflect evolving patient needs.
• Educates patient and family about disease, prevention, palliation and safety.
• Reports changes in condition to IDT or other health professionals.
• Participates with IDT to evaluate referrals/admissions for level of care.
• Delivers concise oral and written reports to IDT and respects input from all disciplines.
• Supervises care by Community Health Workers/Home Health Aides as requested.
• May be required to report to work during emergencies to ensure continuity of services.
