Job summary

Habitat Health in Sacramento is seeking a Medical Assistant to support the PACE center's clinical and administrative needs. You will perform intake, vital signs, phlebotomy, EKGs, immunizations, and documentation as part of an interdisciplinary team.

Collaborating with providers and the IDT, you will help manage participant flow, coordinate follow-up visits, and ensure accurate EMR records while delivering a welcoming, patient-centered care experience.

Qualifications

• High school diploma or equivalent; completion of an accredited Medical Assistant program.
• Current Medical Assistant certification and phlebotomy certification.
• Experience performing routine clinical procedures including intake, vital signs, screenings, and specimen collection.
• Strong communication and collaboration with participants, caregivers, providers, and the IDT.
• Ability to learn EMR systems and maintain accurate documentation in fast-paced settings.
• Active CPR/BLS certification or ability to obtain within 30 days of hire.

Responsibilities

• Support efficient participant flow by intake, rooming, vital signs, screenings, and preparing for examinations.
• Perform laboratory testing, phlebotomy, EKGs, immunizations, and injections as directed.
• Coordinate with providers and the IDT to communicate observations and changes in status.
• Schedule follow-up visits and coordinate clinical activities to ensure continuity of care.
• Serve as first point of contact for participant questions via phone or messages; escalate as needed.
• Maintain accurate intake and clinical documentation in EMR and other systems.
• Assist with referrals, medical record management, and authorizations to facilitate timely care.
• Maintain clinic readiness; assist with room prep, supplies, and equipment.
• Contribute to a welcoming, participant-centered experience and continuous process improvements.
