Job summary

MorseLife Health System is seeking an Intake Referral Specialist to support the daily operations of the PACE referral process. The role includes clerical and administrative duties to keep the office running smoothly.

You will screen and refer patients, manage calls and communications with families, and enter demographics into CRM systems while coordinating with medical offices to obtain records and ensure eligibility.

Qualifications

• High school diploma or GED; 1-3 years related experience and/or training; equivalent combination of education and experience.
• Familiar with Microsoft Office suite and standard computer functions.
• Must have at least one year of experience working with frail elders.

Responsibilities

• Receive and screen referrals.
• Monitor voicemail and return calls to prospective participants and their families.
• Send program information to prospective participants.
• Enter referral demographics into CRM and care management systems.
• Verify Medicaid eligibility through the Florida Medicaid Management Information System.
• Maintain and update spreadsheets for census tracking and state reporting.
