Job summary

Capital Health is seeking a capable Registered Nurse to provide home health visits for LIFE program participants. You will perform assessments, triage, administer medications, and educate patients and families in home and clinic settings.

The role emphasizes safety, collaboration with the care team, and coordination of DME and supplies. A BSN is preferred, and NJ or NLC multi-state licensure is required; BLS certification is mandatory.

Qualifications

• Associate degree in Nursing or graduate from an accredited school of nursing; BSN preferred.
• AHA BLS – Healthcare Provider required.
• Driver's license is required.
• RN license valid in NJ or multi-state license (NLC).
• Home health care experience is preferred.

Responsibilities

• Deliver quality care to patients through the nursing process (assessment, diagnosis, planning, implementation, evaluation).
• Ensure patient safety by following policies and procedures.
• Perform nursing assessments, triage, medication administration, wound care, treatment, and participant education.
• Conduct comprehensive nursing assessments in home and clinic settings; update as status changes.
• Collaborate with the PACE care team via verbal and electronic communication.
