Job summary

The VIOS Clinic is seeking a Registered Nurse (RN) for a Home Health role delivering patient-centered care through in-home visits, telehealth, and essential administrative tasks.

You will conduct comprehensive home assessments, review medications, perform physical exams, administer medications, manage wound care, and educate patients and caregivers while coordinating with an interdisciplinary team to optimize care plans.

Qualifications

• Active RN licensure in the appropriate state.
• Minimum 3 years of direct patient care experience.
• CPR certification.
• Experience with telehealth platforms and EHR systems.

Responsibilities

Job description

Role:

A Registered Nurse (RN) in Home Health providing patient-centered care through in-home visits, telehealth consultations, and administrative tasks.

Responsibilities:

Conduct comprehensive home safety evaluations, obtain patient histories, review medications, perform physical assessments, and deliver clinical interventions such as medication administration and wound care. Document patient interactions accurately, educate patients and caregivers, support care transitions, participate in interdisciplinary team meetings, and collaborate with healthcare providers to develop personalized care plans.

Qualifications & Skills:

Active RN licensure, minimum 3 years of direct patient care experience, CPR certification, and proficiency in telehealth platforms and EHR systems. Prior home care, hospice, or geriatrics experience is preferred. Must possess strong communication, organizational, and decision-making skills, with the ability to work independently and travel locally to patient sites.

Additional Details:
