Capital Health is seeking an experienced RN leader for the LIFE program, responsible for coordinating participant care across the continuum and ensuring quality outcomes. The role requires collaboration with physicians and an interdisciplinary team to manage plans of care, documentation, and transitions of care.

Qualifications

• BSN required.
• 4 years nursing experience required.
• Prior home care experience preferred.
• Prior management experience preferred.
• Demonstrated strong leadership skills required.
• Experience as a case manager, care coordinator or discharge planner preferred.

Responsibilities

• Organizes, coordinates, and delivers participant care using the Interdisciplinary Team process.
• Collaborates with Primary Care Physicians and IDT on care issues.
• Monitors care quality and ensures metrics meet targets.
• Reviews and updates plans of care with participants; intervenes to improve care.
• Ensures complete and accurate documentation; assesses LOC needs and changes.
• Manages transition of care and coordinates with other agencies for appropriate level of care.
• Ensures integrated care plans across disciplines; communicates transition plans clearly.
• Secures necessary services and supplies; prevents gaps in care.
• Leads routine meetings with contracted facilities and IDT; participates in PI activities.
• Performs quarterly reviews of contracted facilities' plans of care; coordinates with LIFE Care Plan.
• Identifies and resolves facility-related issues with LIFE and facility staff.
• Supports IDT in resolving acute complex issues with necessary resources.
• Participates in daily huddles and morning meetings; performs other duties as assigned.
